Abstract:  We are a multidisciplinary therapy clinic that provides services to habilitate children and rehabilitate adult upper extremity (hand/arm/shoulder) conditions. Our goals are to faciliatate independence and enhance an idividual's work & leisure activities and to increase social participation. We strive to unlock human potential and optimize human performanceto allow individuals to assume or regain valued life roles.

Abstract:  Evergreen’s Rehabilitation Services span a number of different programs, including both inpatient and outpatient therapy, the Acute Rehabilitation Unit, Home Health Services and Cardiac Rehabilitation. The Head Injury and Rehabilitation Center provides therapy and neurological services for patients recovering from stroke or traumatic head injury. The Booth Gardner Parkinson’s Care Center provides a complete range of therapy for those dealing with Parkinson’s disease or other movement disorders. A referral from a physician or allied health care provider is required to begin rehabilitation services.

  Abstract: Quality Care Physical Therapy offers support to help restore function, improve mobility, alleviate pain and reduce or prevent dysfunction often sustained from various injuries.
